LIST OF SSA ACCEPTABLE DOCUMENTS

FOR SOCIAL SECURITY CARD REQUESTS

 

In order to prevent fraud and protect you from Identity Theft, the Social Security Administration needs to ensure that you are who you claim to be, making certain that no one may be able to request a Social Security Card with your name on it. This is for your own protection.

That is why you need to present original documents when applying for new or replacement card.

All documents will be returned to you after your card request has been processed.

 

For Adults born in the U.S. –  Card Replacement & Personal Updates

Important

You must present original documents or copies certified by the agency that issued them. The Social Security Administration cannot accept photocopies or notarized copies. All documents must be current (not expired). The S.S.A. cannot accept a receipt showing you applied for the document.

You can order a Driver’s License replacement card or State Issued ID replacement card by visiting your local DMV online.

Identity

  • U.S. driver’s license
  • State issued non-driver I.D. card
  • U.S. passport

If you don’t have any of the above, the Social Security Administration will ask to see other documents. The following documents may be accepted: (Any documents submitted must be current (not expired) and show your name, identifying information – date of birth or age – and preferably a recent photograph).

  • Employee Identification Card
  • School Identification Card
  • Health Insurance Card (not a Medicare Card)
  • U.S. Military Identification Card.

 

Adults born in the U.S. –  Name Change

Please send one of these documents below. If the document doesn’t have sufficient information, such as your old and new name, please send also another form of I.D.

  • Marriage document;
  • Divorce decree;
  • Certificate of Naturalization showing the new name; or
  • Court order approving the name change.

 

For Children born in the U.S. under 12 years old – 1st card,

Important

Anyone age 12 or older requesting an original Social Security number must appear in person for an interview.

For 1st card application please send one from each list, to prove citizenship and identity.

*Age

If your child has or can obtain a U.S.-State-Issued birth certificate that recorded his or her birth before age 5, you must submit it.

For Children born in the U.S. any age – Card replacement (only needs ID)

For Foreign Born U.S. Citizen – Card Replacement, Updates, Name Change

*for name change add one of  the documents from the Name change list.

Important

If you already updated the S.S.A. regarding your citizenship status, you only need to send one form of ID; if not, you will need to send your citizenship certificate also.

Example: You were not a U.S. citizen when you first applied for your social security card, please update your status by sending your citizenship certificate as well.

For Noncitizens

Important

You must present original documents or copies certified by the agency that issued them. The Social Security Administration cannot accept photocopies or notarized copies. All documents must be current (not expired). The S.S.A. cannot accept a receipt showing you applied for the document.

You need to send one (1) from each list, to prove Immigration Status, Work Eligibility, and Identity.

 

  • Immigration status

To prove your U.S. immigration status, you must show the S.S.A. your current U.S. immigration document, such one of these:

  • Form I-551 (Lawful Permanent Resident Card, Machine Readable Immigrant Visa);
  • I-766 (Employment Authorization Document, EAD, work permit); or
  • I-94 (Arrival/Departure Record) or admission stamp in the unexpired foreign passport.

If you are an F-1 or M-1 student, you also must show us your I-20 (Certificate of Eligibility for Nonimmigrant Student Status).

If you are a J-1 or J-2 exchange visitor, you must show us your DS-2019 (Certificate of Eligibility for Exchange Visitor Status).

 

  • Work eligibility

I-94 (Arrival/Departure Record) or admission stamp in the unexpired foreign passport showing a class of admission permitting work. Some foreign workers must show their I-766 (Employment Authorization Document, EAD, work permit) from DHS.

The S.S.A also needs to see evidence of that employment, such as a recent pay slip or a letter from your employer. Your supervisor must sign and date the letter. The letter must describe:

Your job;

Your employment start date;

The number of hours you are, or will be, working;

and your supervisor’s name and telephone number.

Student: If you are an F-1 student and eligible to work on campus, you must provide a letter from your designated school official that:

Identifies you;

Confirms your current school status; and

Identifies your employer and the type of work you are, or will be, doing.

If you are an F-1 student authorized to work in curricular practical training (CPT), you must provide us your Form I-20 with the employment page completed and signed by your designated school official.

If you are an F-1 student and have a work permit (I-766) from DHS, you must present it.

If you are a J-1 student, student intern or international visitor, you must provide a letter from your sponsor. The letter should be on sponsor letterhead with an original signature that authorizes your employment.

 

  • Identity

Social Security will ask to see a current DHS document. Acceptable documents include:

  • I-551 Permanent Resident Card;
  • I-94 Arrival/Departure Record with unexpired foreign passport or admission stamp in the unexpired foreign passport; or
  • I-766 Employment Authorization Document, (EAD, work permit) from DHS.
  • U.S. driver’s license
  • State issued non-driver I.D. card

The SSA will only accept Original Documents or copies certified by the Issuing Agency.
